AQdirect is committed to protecting the privacy of patient information and to handling your personal information in a responsible manner in accordance with the Privacy Act 1988 (Cth), the Privacy Amendment (Enhancing Privacy Protection) Act 2012, the Australian Privacy Principles and relevant State and Territory privacy legislation (referred to as privacy legislation). This Privacy Policy explains how we collect, use and disclose your personal information, how you may access that information and how you may seek the correction of any information. It also explains how you may make a complaint about a breach of privacy legislation. This Privacy Policy is current from 2018. From time to time we may make changes to our policy, processes and systems in relation to how we handle your personal information. We will update this Privacy Policy to reflect any changes. Those changes will be available on our website and in the practice


We collect information that is necessary and relevant to provide you with medical care , specifically your anaesthetic care and post operative care. This information may include your name , date of birth, gender, health information, family history, mobile phone and email address . AQdirect facilitates the process of completing an online anaesthetic questionnaire which has been requested by your anaesthetist. The privacy of this process is protected by SSL encryption technology.

AQdirect has a deliberate policy not to store this information on the web. Your submitted questionnaire containing your name , contact details and health summary is  emailed , one to your requesting anaesthetist and another to yourself.

All doctors registering with AQdirect Pty are required to sign an agreement that;

They will handle received questionnaires , and any other correspondence as a result of the questionnaire in Accordance with the Privacy Act 1988 and all current amendments

  • Specifically AQdirect requires that adequate security is maintained for electronically stored questionnaires. Ie Internet security,access , passwords regularly changed.

  • The questionnaire is for the use of the requesting practitioner and is not to be passed on without the express permission of the patient.

  • The requesting doctor may use the questionnaire for entry into the hospital anaesthetic record.


The Aqform questionnaire can be repeated if data is incorrect. The actual form once submitted is unalterable.
